Lumbar Epidural Steroid Injections

Lumbar epidural steroid injections are minimally invasive procedures used to treat nerve-related pain that travels from the lower back into the buttock or leg, often referred to as sciatica. This type of pain can develop when a spinal nerve becomes irritated or compressed, commonly from a herniated disc or other changes in the lower spine.

The injection delivers anti-inflammatory medication close to the affected nerve to reduce inflammation and relieve radiating pain.

During the procedure, the treatment area is numbed and a thin needle is carefully guided into the epidural space surrounding the spinal nerves. Live X-ray guidance (fluoroscopy) is used for precise placement.

Corticosteroid medication is then delivered near the irritated nerve, often along with a local anesthetic. By reducing inflammation around the nerve, the injection may help decrease pain traveling into the buttock or leg.

Lumbar epidural steroid injections are most commonly considered for patients with sciatica or other radiating leg pain caused by spinal nerve irritation, particularly when symptoms continue despite treatments such as medication, activity modification, or physical therapy.

They may also be considered in selected patients with spinal stenosis when nerve compression is contributing to symptoms.

What to Expect

Lumbar epidural injections are performed on an outpatient basis, and patients return home afterward. Some temporary soreness around the injection area may occur.

The local anesthetic may provide temporary relief soon after the procedure, while the corticosteroid takes more time to reduce inflammation. Improvement may develop gradually over the following days, and the amount and duration of relief varies from person to person.

Reducing radiating nerve pain can make it easier to walk, exercise, participate in physical therapy, and return to everyday activities. For some patients, an epidural injection can provide a period of relief that allows them to make greater progress with rehabilitation and other parts of their treatment plan.
